• DULY ACCOMPLISHED APPLICATION FORM.
• BIRTH CERTIFICATE (N. S.O)-SINGLE
• MARRIAGE CONTRACT (N.S.O)-MARRIED

• SUPPORTING DOCUMENTS TO PROVE IDENTITY(2 VALID I.D'S)-PRC/IBP I.D,DRIVERS LICENSE,SSS I.D,COMPANY I.D,SCHOOL I.D)*if u  only have 1 valid i.d, u still need to provide supporting documents; Voter's Registration Record from Comelec / N.B.I Clearance/ T.O.R/ others..
• FOR MINOR APPLICANT(BELOW 18 YRS.OLD)

• PERSONAL APPEARANCE(of either parent,Marriage Contract and proper I.d of parents)-if minor is of legitimate status
• Birth certificate (N.S.O)
• Document of Identity (for 1st time applicant)
• If both parents are Abroad:

1. Affidavit of Support and Consent from parents indicating the name of traveling companion and must be authenticated by the nearest phil. embassy or consulate general if not executed before a consul.
2. Special Power of Atty.( must be authenticated by the nearest phil. embassy)
3. Passport and photocopy of passport of travelling companion.
