Affidavit of Support and Consent from parents indicating the name of traveling companion and must be authenticated by the nearest phil. embassy or consulate general if not executed before a consul.
Special Power of Atty.( must be authenticated by the nearest phil. embassy)
Passport and photocopy of passport of travelling companion.
