PERSONAL APPEARANCE(of either parent,Marriage Contract and proper I.d of parents)-if minor is of legitimate status
Birth certificate (N.S.O)
Document of Identity (for 1st time applicant)
If both parents are Abroad:

Affidavit of Support and Consent from parents indicating the name of traveling companion and must be authenticated by the nearest phil. embassy or consulate general if not executed before a consul.
Special Power of Atty.( must be authenticated by the nearest phil. embassy)
Passport and photocopy of passport of travelling companion.
